Annual Diabetes Check

You will be invited for your annual diabetes review during your birthday month.

When you receive your invitation, please book a blood test appointment. On the day of your blood test, please bring a urine sample so we can check your ACR (Albumin-to-Creatinine Ratio).

After your blood test, please book an appointment with the diabetes nurse for approximately two weeks later. This will allow enough time for all of your test results to be received and reviewed before your appointment.
